Ozone (O 3); is an unstable molecule in the stratosphere of the atmosphere consisting of three oxygen atoms having a vital task with the blocking of the sun’s ultraviolet B and C rays. Ozone in this layer is constantly changing. Its disparity varies according to the situation. Even a 1% reduction causes a 10% increase in skin cancers. Living creatures live in the troposphere, 10% of the ozone is found in this layer. In the air we breathe, Nitrogen (78%), oxygen (20, 9%), carbon dioxide (0,034%) and argon (0,9%) gases, which are necessary for the living, are found. As the ozone in the troposphere is at a higher level, it does not exist in the breathing air. Ozone was discovered by Austrian chemist Christian Schönbein in 1840. Ozone, an unstable molecule, is gaseous at room temperature and has a colorless, distinctive odor. It is named as active SUPEROXYGEN as it can be easily transformed into stable molecules in the reaction, the use of medicinal purposes stems from this feature. Triatomic molecule of ozone is converted to diatomic molecules of oxygen and single atomic reactive ozonoid (OR). Ozone gas is generated with the help of devices for medical purposes. When normal air is used, N2O2 (nitrogen dioxide) is formed. Half-life of it is 40 minutes at room temperature. Indeed, it is the production done using with the oxygen cylinders producing 0.05 -100 µg/ml concentrations by determining doses according to the application area.

Ozone is in the service of humanity on sterilization industry and the food and in the veterinary field for many years. It is used for sterilization in these areas. FDA has given approval of its use in food sterilization in 1982. Ozone was used in the sterilization of water in the 2nd World War in Germany. As it is about 3000 times stronger disinfectant than chlorine, it is used against parasites, spores, fungi and cysts in pool. It is used for prevention of contamination of food (transmission) and shelf-life extension in the livestock sector. It is used in the medical world for 50 years. In Turkey, while previously applied unofficially, it was included in certified education by the Ministry of Health with the regulation issued in 2014; doctors and dentists were given practical training at some universities.

Effects of Ozone:

Events which are the basis of disease in our bodies begins with the domino effect. Inflammatory process begins with insufficient oxygenation of tissues and organs. With the start of this process, acidosis, resulting in the inflammatory reactions, develops in organs. The main problem of arthritis table is that edema, swelling and redness is formed due to the inflammation caused in the damaged area. In autoimmune diseases, the issue in the etiology is the clinical conditions which are caused by autoantibodies generated against the respective structures of the body. Two treatments methods were determined based on the clinical condition of modern medical understanding. The sense of suppression and the replacement approach. Replacement therapy is applied for replacing the hypothyroidism that is generated as a result of the Graves’ disease. In almost all of the rheumatic diseases suppression method is applied using different mechanisms. Therefore, it is impossible to resolve these diseases basically. NSAIDs and corticosteroids are the most important drugs, there is no chance of these drugs to treat, and their side effects are too many on the contrary. Therapies used in patients with arthritis cannot go beyond the local and general NSAID drugs.

The basic mechanism separating Integrative (holistic) treatment methods from common treatment approach is this. Like in all these methods the effect in ozone treatment is not suppression and the replacement approach but to remind the essential tasks of organs and tissues. The basic approach separates here. To give fish every day does not solve the problem. As he knows the fishing, you need to remind him again. Circulatory disorders that occur for various reasons disrupt the oxygenation rapidly. If the circulation cannot be held and oxygenation cannot be achieved, then the acidosis and consequent suppression of inflammation and widespread pain will be inevitable. The main effect in the ozone treatment is to ensure the effective oxygenation level by regulating the circulation and to help repair the disease by activating immune system generating free radical. Responses generated by the ozone treatment are summarized as follows.

Dose-effect and toxicity Relations

There are two major problems in the application of ozone. These are the inadequate dose therapy and the insufficient information to set the disease in specific dose for patient. Ozone treatment is no different from drug-dose relationship. The important thing is, after determining specific dose and method, to put it into practice by skilled person. Otherwise, the society will lose the confidence for the applications. The high dose of ozone therapy has side effects. The most important side effect is hemolysis. Risk of hemolysis is more at high doses. The principle of treatment is starting with low doses and increasing gradually.

Application Methods:

We can divide it into three as application methods for general (systemic) applications, local applications and combined applications. While Intravenous, intra-arterial, lymphotropic, rectal, sauna, MAH-GCD and SFA methods are systemic methods; dermal, intralesional, intracavitary, intra-operative dental procedures are topical applications.

Ozone therapy has also use in combination with other treatment methods. These methods are PRP-CGF, the Prolozone, colon hydrotherapy, acupuncture, hyperbaric oxygen therapy and phytotherapy. The areas in which Ozone therapy is successful are limited in individual applications. The combined use of treatment methods that we believe successful on treatment and was proven by studies has always been more useful and our clinical experience has shown us that. Therefore, our clinical benefit from the use of integrative methods together would be more appropriate in terms of treatment. The common individual application methods listed below.

Major Autohemotherapy

50-300 cc of blood is taken from the patient, up to 750 cc of blood can be taken in 2 hours. The amount of blood taken daily should not exceed 3 liters. After the suitable doses of ozone obtained by device mixed with blood, must be injected not later than two hours.

External Treatment

It is applied by circulating ozone gas in a closed system in a special plastic boots or with the aid of placing proper bags, foils on different parts of the body. This method is used in, ulcers, non-healing wounds, infected lesions in the postoperative and treatment of chronic dermatitis.

Rectal use of application

It is a method which is difficult for the patient to accept. In fact, patients do not feel absolutely no discomfort, because the O3 gas is directly absorbed by the sensitive intestinal mucosa. This method is indicated in inflammatory bowel disease. However, as it is less invasive, it is used for general health and revitalization recently.

Intra-Articular Injection

Ozone gas is slowly injected into the joint. It is the most important application areas of ozone that requires expertise. This method is applied in inflammatory arthritis. Applying in infection related septic arthritis may be objectionable. Septic arthritis is a condition that should be treated with urgency. For this reason the detection if the arthritis table is infection related or inflammatory is important to prevent permanent damage.

Ozonized oil

I is obtained through long (45 days) the implementations of pure olive oil with the advanced techniques. It is applied on dermatitis in the form of eczema, the seborrheic dermatitis, fungal infections and at lichen planus.

Acute Hemorrhagic Disease (acute period of MI, CVA) patients with G6PD deficiency, in the first three months of pregnancy, in the fields of ACE inhibitors and anticoagulants, in Hyperthyroidism, Thrombocytopenia and severe bleeding, on Transplant patients its  implementation is objectionable.

Side effects:

The most important side effect is hemolysis. Hemolysis is the release of content formed with the breakdown of red blood cells into the plasma. It is caused by error made in dose adjustments of the treatment. Even at therapeutic doses hemolysis may develop at the rate of 0.4-1, 2%, however, this is a ratio that can be tolerated. At high doses, the ratio may be up to 7%. In patients with chronic renal failure and heart failure, hemolysis can cause serious consequences. Determination of the appropriate dose and treatment to the convenient patients by experts is important in this respect. As it is an invasive process, treatment method must be approached with sensitivity. Another side effect is the air embolism, its risk is greater in local applications.
